Elements Support Services is a specialist rehabilitation service meeting the needs of children, youth, and adults with a brain injury or other serious trauma. Working in coordination with the Rehabilitation Team our support is instrumental and focused on the individual reaching their full potential.

Our Services

 

Community and Home Rehabilitation

We meet the needs of people at different stages of the rehabilitation process so that they can regain lost skills and independence. Working within the home setting and within the community, we maximize skill acquisition through a deliberate process of following the recommendations of the Rehabilitation Team. We make the process pragmatic so that it feels natural as the individual learns and reacquires skills.

Intensive Assessment Service

Our individualized program is custom designed for each client. We work with the community treating Rehabilitation Team and implement a comprehensive living assessment utilizing a community-based environment. During this time, we collect extensive data to support the treating team in completing an in-depth assessment of the client’s functional, psychosocial, behavioural, and cognitive skills. Our program is intensive and utilizes the assessment results to create real, meaningful and functional programs.

Residential and Inpatient Support

We provide Residential Programs and Hospital In-Patient Programs with supplementary, experienced staff to support their programs in meeting the needs of the people that they serve. Services are available on an as needed basis for short-term and long-term coverage.

Post Rehabilitation Support

Once the active rehab phase ends, many individuals still need some level of support. Our Support Workers foster long-term, stable relationships with the Brain Injury Survivor and offer the kind of support needed to manage residual impairments, day-to-day living issues and maximize the quality of life.
